1. A copy of each applicant's travel itinerary or a company letter with your itinerary on it, showing your dates of travel and your travel destination on company letterhead and signed by someone other than the applicant
   
2. A copy of Bucktown Visas & Passports Letter of Authorization.  The signature on this letter must match the signature on your passport application.
   

**NOTE: Effective immediately, passport photo with persons wearing eyewear will be rejected. Please remove your eyeglasses before having your photos taken. Thank you for your cooperation. 

Passport Renewal or Passport Name Change

*To make it easier to follow, you may want to print out this page and then check off each item on the list as you compile your documents.

You do not need to appear in person at a government acceptance agency in order to renew your passport or to change the name on your passport.

• Your current United States passport, signed
  

Passports may only be renewed if your passport was issued when you were 16 or older, and your prior passport is still in your possession, and your prior passport was issued less than 15 years ago. You have within 5 years to renew your passport from its expiration date. If you do not meet these requirements, follow the instructions for a New U.S. Passport. If you have lost a valid passport, follow the instructions for a Lost Passport. Damaged passports may not be renewed.

• 1 passport photos, 2 x 2 inches in size with full, front view of your face, with light or white background. Sorry, no digital photos allowed!

**NOTE: Effective immediately, passport photos will be rejected by The State Department if the person is wearing any type of eyewear. Please remove your eyeglasses for your passport photo. Thank you for your cooperation.

You can have them taken at a CVS Pharmacy or Walgreens drugstore.  Do not staple the photo to any form.

• Passport renewal government form DS-82 (click here to print out a DS-82).

Filled out completely and signed.  You must fill out the application online and then print it out and sign it. Please do not forget to sign it!  Include all pages of Form DS-82.

• 2 Bucktown Visas & Passports Letters of Authorization, both filled out and signed.  The signature on these two letters must match the signature on your passport application

• Your original marriage certificate.
• Your original divorce decree.

• Government fee of $209.53 For your convenience, there are 2 ways to pay. (*By law, this fee is nonrefundable.)

1) You can write a personal check, money order, or cashier's check payable to "U.S. Department of State" for the amount of $190.00.
2) You can use a credit card, money order, or company check made out to us (payable to Bucktown Visas & Passports, Inc.) in order to pay for the full amount of your government fees and your Bucktown Visas & Passports expediting and return shipping fees with just one simple payment.

• Bucktown Visas & Passports Passport Order Form, filled out completely.

Click on the "Visa/Passport Order Forms" button on our menu to print one out. It has our expediting and shipping fees on it, which are in addition to your government fees.

Shipping: Please send the following documents by FedEx envelope to Bucktown Visas & Passports at the address located on the top of our order form:

1. Your current United States passport, signed
2. 1 passport photo (Photos with persons wearing any type of eyewear will be rejected by The State Department.)
3. DS-82 government passport application, filled out online, printed out, and then signed. Please do not forget to sign it!
4. 2 Bucktown Letters of Authorization, signed The signature on these two letters must match the signature on the passport application.
   
5. (If changing names, original marriage certificate or divorce decree)
6. Government fee of $209.53, (payable by either of the options listed above)
7. Bucktown passport order form
8. Bucktown expediting and shipping fees located on our order form. They are in addition to your government fees.

We will notify you by email that we have received your documents, and we will also email you when we ship out your passport to you.
