Second Passport Requirements:

*To make it easier to follow, you may want to print out this page and then check off each item on the list as you compile your documents.

• Your valid United States passport, signed
  

If you have lost a valid passport, follow the instructions for a Lost Passport. Damaged passports may not be renewed.

• 1 passport photos, 2 x 2 inches in size with full, front view of your face, with light or white background. Sorry, no digital photos allowed!

Effective November 1, 2016, passport photos with persons wearing any type of eyewear will be rejected by The State Department.

You can have them taken at a CVS Pharmacy or Walgreens drugstore.  Do not staple the photo to any form.

• Passport renewal government form DS-82 (click here to print out a DS-82).

Filled out completely and signed.  You must fill out the application online and then print it out and

sign it. Please do not forget to sign it!  Include all pages of Form DS-82.

• For a second passport, you need to fill out a different form, in addition to the DS-82 listed above. Please click here for the second passport form.

• Bucktown Visas & Passports Letter of Authorization, filled out and signed.  
• The signature on this letter must match the signature on your passport application. Click here

• Proof of travel departure date (include one of the following):
• Print out a copy of your travel itinerary
• A company letter of expedite (must have company's letterhead on it, indicate the date you will be traveling and to what destination, and must be signed by someone other than the applicant.)

• Government fee of $210.00. For your convenience, there are 2 ways to pay. (*By law, this fee is nonrefundable.)

1) You can write a personal check, money order, or cashier's check payable to "U.S. Department of State" for the amount of $190.00.
2) You can use a credit card, money order, or company check made out to us (payable to Bucktown Visas & Passports, Inc.) in order to pay for the full amount of your government fees and your Bucktown Visas & Passports expediting and return shipping fees with just one simple payment.

Shipping: Please send the following documents by FedEx envelope to Bucktown Visas & Passports at the address located on the top of our order form:

1. Your valid United States passport, signed
2. 1 passport photo (Effective November 1, 2016, passport photos with persons wearing any type of eyewear will be rejected by The State Department.)
3. DS-82 government passport application, filled out online, printed out, and then signed. Please do not forget to sign it!
4. A different application, which is in addition to the DS-82 mentioned above. Click here for that.
5. Bucktown Letter of Authorization, signed The signature on this letter must match the signature on the passport application
6. A copy of your travel itinerary or company expedite letter
7. (If changing names, original marriage certificate or divorce decree)
8. Government fee of $210.00 (payable by either of the options listed above)
9. Bucktown passport order form
10. Bucktown expediting and shipping fees located on our order form. They are in addition to your government fees.

We will notify you by email that we have received your documents, and we will also email you when we ship out your passport to you.
